[Mediawiki](http://mediawiki.org) is a good tool to create large wikis. For smaller wikis, SimpliWiki is easier to use.

You don't need to. However, you may, if you want, connect your wikis with your profiles on Facebook, Twitter, etc. It makes it easier for you to get back to your wikis when you comeback, see sign in.
